A later randomized, double-blind controlled study in 49 patients aged 45–90 (Debbi et al., 2011, PMID 21708034) used MSM at 1.125 g three times daily and reported improvements in WOMAC pain and function scores versus placebo.

A 2011 systematic review pulling together six randomized trials concluded that MSM produces moderate but real reductions in pain and physical impairment in mild-to-moderate osteoarthritis, with no serious adverse events at doses up to 6 grams per day.

The honest state of the science

MSM’s evidence is more encouraging than glucosamine’s or chondroitin’s when it comes to consistency across trials — but let’s be clear-eyed. Most studies are relatively short (around 12 weeks), the participant numbers are modest, and “moderate improvement” is not the same as a cure. MSM won’t rebuild worn cartilage or reverse osteoarthritis. What the research supports is a real, if modest, reduction in pain and stiffness for many people — which, for a safe and inexpensive supplement, is a reasonable proposition.

Why MSM Is Usually Paired With Glucosamine

You’ll rarely find MSM sold alone in a joint formula — it’s almost always combined with glucosamine and often chondroitin. This isn’t random. It’s because they work through different mechanisms, and the research suggests they may be better together.

One frequently cited trial (Usha & Naidu, 2004) compared four groups in 118 people with knee osteoarthritis: MSM alone, glucosamine alone, the two combined, and placebo. All three active treatments helped — but the combination of glucosamine and MSM produced greater improvements in pain and swelling than either ingredient on its own.

It makes intuitive sense: glucosamine supplies cartilage building blocks, while MSM calms inflammation and provides sulfur. They’re covering different bases. This is exactly why the best joint supplements tend to include all three together rather than betting on one.
